After submitting feedback, your submission will be reviewed by the appropriate department leader within 5-10 business days. If you provide contact information, you may be contacted about your submission.

Feedback submitted here will be accessed by the Wellness Center Director of the college you selected and by the Executive Director of Student Wellness at district office. Compliments and complaints information can also be accessed by CCC IT staff and administrators. If you would like to submit a confidential report, please call or visit the Wellness Center Director for your college.
