We hope this message finds you well. As a recipient of the Chicago Early Learning Workforce Scholarship (CELWS), we are excited to hear about your work experience since completing your studies.

As a reminder, one of the commitments of the CELWS is to work at a Chicago Early Learning (CEL) program (birth–Pre-K) for three consecutive years following your graduation or program completion. To ensure this requirement is being fulfilled, we ask that you complete this brief survey at your earliest convenience. PLEASE NOTE: You will receive this form annually until your work commitment has been completed.
