Welcome! Please view all videos linked and answer all questions on this form. Once you have completed the onboarding quiz section, please continue to agree to all CELWS commitments and sign and submit this form. You will have 7 calendar days to complete this form, in order to move forward as a scholar.

Note: The CELWS Onboarding Quiz & Commitments form contains eight sections. Click ‘Next‘ at the bottom of each section to proceed. On the final section, click ‘Submit‘ to complete and submit the form. Failure to submit this form may delay consideration for the scholarship.

To complete this form, you are strongly encouraged to utilize a computer or tablet.

1. Please view the linked videos below and answer the corresponding questions. All questions and statements in this form require a response. Please ensure that you are thoroughly responding to all questions and statements as you will not be able to continue or submit the form if something remains unanswered.

2. If using a cellular device, please click and hold the linked videos to open them in another tab in your browser (this will happen automatically if you are using a computer or tablet and may also happen on your phone). Click and access the video to watch it in its entirety.

3. Upon viewing the video, access your tabs to return to the form. You are encouraged to close out of any open tabs with videos you have already viewed.
